
Grant (Overview)
The treatment includes services for one Fresh IVF cycle as follows:
1. One egg retrieval
2. Fresh Embryo Transfer (FET) (if embryos are developed)
3. Fertilization of all eggs (with or without ICSI)
4. Storage for 1 year (after freezing embryos – if extra embryos)
Requirements
Ages 21 - 38
with a regular menstrual cycle – an exception will be made for women with PCOS under 38 years of age
No active fertility coverage
If active fertility coverage from insurance you will not be eligible
All Treatments must be completed within 30 days
Household income under $60,000
Proof Required
Same Sex Couples w/Donor Sperm
Sperm purchased separately – also eligible

Included in Grant

All bloodwork and ultrasounds monitoring for the IVF cycle are included once the stimulation medications have been started.

One egg retrieval ONLY is included.

Fertilization of all the eggs with or without ICSI (If there are eggs collected) is included.

Embryo culture is included.

One fresh embryo transfer ONLY (If embryos are developed) is included.
Not Included in Grant

Medications are not included, and they are the patient's responsibility. They are bought from the pharmacy.

Grant does not include anesthesia- sedation (local or IV).

Freezing and storage for any extra embryos are not included in the grant money.

If there are no eggs at retrieval and/or if embryos do not develop, the Grant Cycle is completed.

PGT (genetic testing of embryos) is not included.

Any future treatments (such as Frozen Embryo Transfer) are not included.

Any other testing (such as screening tests) and services (such as saline ultrasound, hysteroscopy, endometrial biopsy…etc) that might be required during the IVF cycle are not included, can either be billed to participating insurance or will be an additional charge.
Important: This is a one-time acceptance, and the couple will be exempt from any other Grant promotions in the same year.
