$50 fee will be charge when a check bounces or a credit card dispute is filed.

Patients who pay with a credit card will be charged 8% service fee on any refund requests

I understand that full payment for services rendered by RFC is due upon the start of cycle. No portion of a cycle will be refunded. In the event on non-payment, a cycle may be cancelled at any time. In the event that my (the patient’s) insurance company denies payment for services rendered, I (the patient) accept full responsibility for the amount denied. It is my (the patient’s) responsibility to verify and know my insurance coverage. I (the patient) have been advised that many insurance companies can and may deny payment for ovarian monitoring (visits, blood test, and sonograms) if the treatment is in preparation for an IVF/egg freezing cycle. It is my responsibility to ask for clarification for anything in this contract that I do not fully understand. Prices are subject to change and are not valid after 3 years from date of signature.
